About Our Founder

Hello, I’m Gia Short, PMHNP-BC, a board-certified psychiatric nurse practitioner and founder of MindSculpt Psychiatry.

I specialize in treating ADHD, anxiety, and depression, with a focus on how these conditions can affect daily life, relationships, and overall well-being. I also have a special interest in binge eating disorder and weight concerns related to psychiatric medications, helping patients find treatment options that support both their mental health and weight-related goals.

My approach is collaborative and patient-centered. I believe every person’s story is unique, and I take the time to listen, understand your concerns, and develop an individualized treatment plan that works for you—not a one-size-fits-all approach.

With more than 15 years of nursing experience, I’ve seen firsthand how the right support can make a meaningful difference. My goal is to help you better manage your symptoms, regain balance, and move toward the life you want with greater clarity and confidence.

Insurance & Self-Pay Options

MindSculpt Psychiatry proudly partners with Headway to provide in-network mental health services for patients throughout Illinois.

If you plan to use insurance, you can verify your benefits, review accepted insurance plans, and schedule an appointment directly through the secure Headway portal.

Self-Pay Appointments (No Insurance)

MindSculpt Psychiatry also offers self-pay services for patients who prefer not to use insurance or who wish to utilize out-of-network benefits.

Self-Pay Fees

Initial Psychiatric Evaluation: $200
Follow-Up Medication Management: $150

Payment is due at the time of service.

A superbill may be provided upon request for patients seeking possible out-of-network reimbursement through their insurance plan.

Self-pay appointments cannot be scheduled through the Headway insurance portal and must be arranged directly with MindSculpt Psychiatry.

Documentation Services

MindSculpt Psychiatry provides documentation services for established patients when clinically appropriate and supported by medical necessity.

Available documentation services may include:

• FMLA Forms
• Disability Forms
• Workplace Accommodation Requests
• School Accommodation Requests
• Other Clinical Letters or Documentation

Please allow 7–10 business days for completion of requested paperwork.

Documentation Fees

FMLA Forms: $75 per request
Disability Forms: $75 per request
Accommodation Letters: $75 per request
Additional Forms or Letters: $75 per document

Documentation fees are not covered by insurance and must be paid prior to completion of requested paperwork.

Completion of documentation does not guarantee approval of leave, accommodations, disability benefits, academic modifications, or other requests. All documentation is completed at the discretion of the provider based on clinical assessment, supporting documentation, and medical necessity.
